I understand why.

I?ve stood in his shoes and listened to family members say they want to be present 24/7 with their loved ones, often in the most challenging of hospital environments, such as the intensive care unit.

And initially ? like many of my colleagues ? I was hesitant.

An ICU room is filled with lifesaving equipment ? ventilators hissing, monitors beeping, red and green lights flashing ? around which doctors and nurses carefully maneuver. If families are there, the nurses must keep a watchful eye to make sure that no equipment is moved. ?Nurses like to be in control of their work environment,? an ICU nurse manager told me.

Moreover, in the ICU, a family member could become distressed to see their loved ones bucking the ventilator or bleeding from a fresh wound.

However reluctant we doctors may feel, ?patient-centered care? ? in which patients and families are involved in shared decision-making ? lies in our future. Over the past year, the ICU at my hospital has been field-testing a more open approach. We are not the first to do so. Geisinger Medical Center in Danville, Pennsylvania, went to an open ICU policy nearly a decade ago, found it extremely disruptive and soon reverted to only 30 minutes of visiting six times a day. On a second attempt, however, Geisinger developed an extensive communication program for both families and staff, and open ICU visitation has been successful since 2003. A 1997 study found that open ICU visitation practices had a beneficial effect on 67 percent of patients and 88 percent of families.

I am surprised by how well the open policy at our hospital has worked over the past six months. I have become comfortable seeing family members stretched on recliners in the ICU during my early-morning visits. They update me on how the night went for the patient. One ICU specialist said, ?I don?t have to chase down families to update them on what is happening.?

Some ICUs are also inviting families to participate when a team of a dozen professionals, including doctors, nurses, pharmacists and social workers, decide on the plan for the patient.

?I?m not sure that is such a good idea,? one cardiologist told me. It was 8:30 in the evening, and he still had not finished seeing his patients.

I understand his reluctance. During rounds, doctors and nurses exchange medical jargon and spew numbers like auctioneers. Would family members understand what it means when we say their loved one ?needs a cabbage? (a coronary artery bypass graft, or CABG)? Would the team hesitate to talk about potential causes of an illness such as HIV or cancer, or to refer to ?an extremely poor prognosis??

I think back 15 years, when my father had his first catheterization and angioplasty. His heart stopped, and he began to scream. I wasn?t there with him, but I?m fairly certain I could have contained myself. But my mother, not a medical person, says she would have found it very upsetting.

His distress, her reaction, my need to comfort them both ? it would all have disrupted the team focused on threading a wire in a clotted artery.

And what if patients and relatives want to photograph or video a doctor visit? One in seven patients experiences an adverse event during a hospital stay. Inviting families to be with their loved one during rounds will mean that lawyers will have a field day, one internist told me.

Steve Bergeson, medical director for quality at Allina Health Systems in Minnesota, told me that doctors and nurses are afraid that ?families will find out that something went wrong and will be upset.?

I asked the ICU nurse manager at my hospital what nurses do when the family is in an ICU room and something goes terribly wrong, like a patient requiring CPR. ?We ask the family to step out,? she explained, ?and if they want to be present in the room, one of the nurses stands to the side with them and explains what is happening.?

In pediatrics and childbirth, we are far ahead in patient-centered care. No one hesitated when I said I wanted to be with my wife for the delivery of our children, even when things went bad and the obstetrician had to pull my daughter out with forceps to find the umbilical cord wrapped around her neck six times.

While most doctors now accept open visitation, having family members present in the cath lab remains a challenge. My request to accompany my father was a further step in the new culture of patient-centered care. One cardiologist told me, ?I would never allow that. . . . The family has to have a level of trust in me as their doctor. They don?t need to be there to watch over me. I earn their trust before the procedure, and if they don?t like it, they can find another doctor.?

Listening to him, I reflected back on my experience. While my father lay on the cath table covered to his chest with a white sheet, his cardiologist hesitantly agreed to have me watch from the glass-walled control room. He made an exception for me because I am a colleague.

I watched as the doctor threaded the catheter up the artery from my father?s groin. In a snakelike movement, the catheter found its way to his chest and then spat out dye, which dissipated into the heart like black smoke and seeped into the clogged arteries.

Then the cardiologist straightened up, took off his gloves and came to the control room. We conferred on the call. ?I think we can do a stent,? he said. I agreed, went over and took my father?s hand and told him the plan. ?Whatever you think right,? he said.

The more I think about it, the more I realize that movement toward greater transparency is a journey each health system, its medical staff and its patients have to make at their own pace. It cannot be mandated by a federal agency.

Health care is not like a business that can be governed by the simple adage that the customer is always right. But those of us who work in that system should think of every patient as an individual and respect his or her needs and wishes ? a goal that I heard most forcefully expressed at a conference about a decade ago:

?Nothing about me, without me.?

The leadership contest is emerging as a close race between Maehara, a youthful defense expert and the public's top choice, and Economy Minister Banri Kaieda, who secured the backing of the ruling party's behind-the-scenes powerbroker, Ichiro Ozawa.

"In principle, we will not build new nuclear plants; then there will be no more nuclear plants in 40 years," Maehara said, adding that Japan needs to seek "the best energy mix" while it phases out of its nuclear-reliant energy policy.

Before the March accident, Japan derived about 30 percent of its electricity from nuclear plants, and the government intended to increase that to 50 percent by 2030 ? a plan that has now been scrapped.

Kaieda, whose minister was broadly responsible for nuclear energy promotion, said he planned to decommission aging nuclear plants found to have problems during stress tests, but did not detail his vision for the future of atomic energy.

He promised to speed up decontaminaton efforts and launch health check programs for concerned residents.

"We will achieve a cold shutdown of the reactors as soon as possible," Kaieda said. "I will take concrete measures to address the residents' concerns about their health."

Kan announced Friday he would resign after serving nearly 15 months that have been plagued by ruling party infighting, gridlock in parliament and clamorous criticism of his administration's reponse to the March disasters and ensuing nuclear crisis.

The Japanese public, yearning for political unity and resolve in the wake of the catastrophe, has grown disgusted with the squabbles and blame-trading that have dominated parliamentary sessions.

"It's embarrassing. It's hard to keep track of who's prime minister these days," said Rie Aoki, a housewife in the Tokyo suburb of Fuchu. "It's so upsetting to see them squabbling in parliament. Elementary students have more interesting conversations."

She said she hoped the politicians would focus on the northeastern coastal region ? called Tohoku ? that was devastated by the tsunami. "I really want them to work together to think about what Tohoku needs," she said.

The five candidates generally agreed during the two-hour press conference that the central government needed to spend more money on disaster reconstruction in the tsunami zone, possibly raising money through special bond sales.

Given the slow economy, all agreed it was too early to consider raising Japan's 5 percent sales tax.

But the gathering also helped bring out some policy differences between the five, which also includes Finance Minister Yoshihiko Noda, considered a fiscal conservative, former Transport Minister Sumio Mabuchi and Agricultural Minister Michihiko Kano.

Maehara said he would favor reaching out to key opposition parties to form a limited "grand coalition" on certain key policies, such as tsunami reconstruction, social security and tax reforms. Maehara also said he would support a U.S.-backed free trade zone called the Trans-Pacific Partnership.

Kaieda, a 62-year-old former television commentator on economic matters, said the so-called TPP needed to be studied more and rejected the idea of a grand coalition, saying the party had not even discussed such a proposal.

A China hawk, Maehara, 49, gained prominence by taking a firm stand toward Beijing during a territorial spat last year over some disputed islands in the East China Sea.

He defended his decision to run despite having quit as foreign minister in March after it was discovered that he had unknowingly received a relatively small political donation from a foreigner ? a Korean permanent resident and a childhood friend ? which is illegal in Japan.

"I don't have anything to hide," he said. "I haven't done anything that tainted my own hands."

(AP) ? Stock indexes fell sharply soon after trading began Thursday and then bounced around near their bottoms the rest of the day, ending a three-day rally.

Indexes in both the U.S. and Europe sank after Germany's main stock index, the DAX, suddenly dipped 4 percent. Traders struggled to explain the dive. The Dow Jones industrial average closed down 170.89 points, or 1.5 percent, to close at 11,149.82. It had been up 85 points the first few minutes of trading.

Bank of America Corp. jumped 9 percent on news that Warren Buffett will invest $5 billion in the troubled bank. BofA had lost half its value this year as investors worried about its need to raise capital and its growing liabilities related to subprime mortgages. BofA stock was up 26 percent early, to $8.80, and closed at $7.65, up 66 cents.

Other banks also rose after the billionaire investor gave his backing to Bank of America. Morgan Stanley gained 2.7 percent and Citigroup Inc. 4.8 percent. BofA and American Express Co. were the only two of the 30 companies in the Dow to rise.

This week's trading has been marked by a series of sudden reversals. Robert Stein, a money manager responsible for $1.2 billion at Astor Asset Management, said questions about the economy have made investors uncertain and the stock market more volatile. Gains made one day have disappeared the next, or even in the same day.

"We're not seeing anything that's convincingly bearish enough to call another recession, but nothing optimistic enough to suggest that a recovery is going to regenerate," Stein said.

Friday could be another day of big swings as Federal Reserve Chairman Ben Bernanke gives a highly anticipated speech at a conference in Jackson Hole, Wyo. Bernanke speaks at 10 a.m. EDT.

Earlier Thursday, the government reported an increase in the number of people applying for unemployment benefits last week. The Labor Department said applications rose to 417,000, the highest in five weeks, but the figure was inflated by a strike at Verizon that earlier this week.

The S&P 500 index fell 18.33 points, or 1.6 percent, to 1,159.27. The Nasdaq fell 48.06 points, or 1.9 percent, to 2,419.63.

The S&P, the benchmark for most money managers, has gained 3 percent this week but is still down 10 percent for the month. Thursday's drop broke a three-day rally in which the Dow gained 503 points.

Paul Zemsky, chief investment officer of ING Investment Management in New York, said this week's gains were a result of investors bargain-hunting after stocks fell too far over the past month. He also said some of the gains were caused by technical trading as investors bought shares to exit short positions, or bets that the market would continue to fall.

Zemsky expects to see more big swings as long as the fear of recession hangs over the market. "People are trying to adjust their positions to news," he said. "Once it's clear where the economy is headed, I think things will calm down."

There's plenty of speculation about whether Bernanke will offer more support for the economy. It was at the same conference last year that he laid out an argument for what became a $600 billion bond-buying program by the central bank.

Zemsky thinks there's little chance Bernanke will announce any action Friday. "There's nothing more than hope that Bernanke will drop a gift from the sky."

More than three stocks fell for every one that rose on the New York Stock Exchange. Trading volume was above average at 5 billion shares, thanks to BofA, which accounted for nearly 900 million shares.

Fossil fish reveals how jaws evolved

Scientists have shown for the first time the reorganisation of the brain and sense organs that needed to happen for jaws to have evolved in our distant ancestors.

They used high energy synchrotron X-rays to look inside the heads of 400 million-year-old fossils of a long-extinct fish called a galeaspid from China and Vietnam.

Their results, published in Nature reveal the exact shape of the brain and sense organs this ancient creature had.

Galeaspids are a type of bony fish with no jaws. Scientists think they're more closely related to today's mammals, birds and fish - and other vertebrates with jaws ? than jawless vertebrates like hagfish and lampreys, and so represent an evolutionary intermediate.

Almost all vertebrates, such as mammals, birds, reptiles and other animals with backbones, have jaws. But some, like hagfishes and lampreys, don't.

How our ancestors went from having no jaws to having jaws ? and a mouthful of teeth ? is one of the biggest mysteries in evolutionary biology, and fossil evidence has provided few insights.

'Evolutionary developmental biologists have said for decades that you'd need a major reorganisation of the head for jaws to develop,' says Professor Phil Donoghue from the University of Bristol, co-author of the study.

'People have looked for evidence of this reorganisation of the head, but haven't found it, so they've suggested maybe there had been an evolutionary jump. But we found that did not happen,' he adds. Instead, for our ancestors to have evolved jaws, the way their brains developed would've had to be reorganised.

Donoghue and colleagues from China, France and Switzerland realised that they would've had to have two nasal organs at the front of the brain instead of one. Indeed, after scanning nearly 40 fossils, they discovered that galeaspid has one huge nostril, but two nasal organs, just like modern jawed vertebrates, while lampreys and hagfishes have just one nostril.

The stem cells that go on to create the jaws have to move forward from the back of the brain towards the front during development of the embryo for jaws to develop. But if you've got your nasal organs clumped in the middle of the face, that can't happen: the nasal organs act like a physical barrier to the movement of these stem cells.

But because galeaspids have two nasal organs with a gap between them, there's room for the stem cells to migrate all the way forward, meaning that it's possible for jaws to evolve in the space available.

Donoghue says this probably happened tens of millions of years before the origin of jaws.

'Our results show that the reorganization of the head to accommodate jaws wasn't a one-time event, the sense organs in the head were being rearranged for other reasons. Nostrils might have separated for feeding and breathing purposes,' he says.

Galeaspid's huge nostril in its head would have let it get more oxygen out of the water, because it would have increased the flow of water across its gills.

'This technology let us see the paths of all the veins, nerves and arteries that would've plumbed the brain of these amazing fossils,' explains Gai Zhikun also from the University of Bristol and the Institute of Vertebrate Palaeontology and Palaeoanthropology in China, lead author of the study.

Importantly, the technique is completely non-invasive, which means that valuable fossils don't need to broken up - unlike using traditional methods.

'This work would have been impossible without using synchrotron technology and our collaboration with the Chinese,' says Donoghue. 'You need access to technology and access to fossils.'

Robust preschool experience offers lasting effects on language and literacy

Preschool teachers' use of sophisticated vocabulary and analytic talk about books combined with early support for literacy in the home can predict fourth-grade reading comprehension and word recognition, new research from Vanderbilt University's Peabody College finds.

The findings, published in Child Development and included in a review article in the August 19, 2011 edition of Science, present evidence that there are lasting, complex and mutually reinforcing effects that flow from strong early childhood classrooms.

"We need to take very seriously the importance of teaching language in the preschool years," said David Dickinson, author of the study and professor of education. "It's easy to look at tangible accomplishments such as counting or letter recognition but much harder to measure richness of vocabulary and language ability. Parents should take a careful look at what is happening in their kids' preschool classrooms and see if the teacher is engaging the child in conversations that are rich in language." This latest research, co-authored by Michelle Porche of Wellesley College, reports results of a longitudinal study which examined in detail language experiences of children from low-income homes when they were in preschool. The authors sought to identify influences of these early experiences on children's language and literacy at the end of kindergarten and again in fourth grade.

Preschool teachers were audio and videotaped, teachers were interviewed and classrooms were observed for their support of language and literacy. Children were individually assessed, and parents were interviewed to learn about their education level and income and any family practices which foster language and literacy.

Although the sample was small, the researchers found robust relations between early classroom support for language and later language and reading ability.

One preschool teacher behavior which predicted children's growth was the frequency of sophisticated vocabulary use during informal conversations. Such exposure predicted children's kindergarten vocabulary, which correlated with fourth grade word reading. Teachers' use of sophisticated vocabulary also correlated with children's kindergarten print ability, and through that word reading skill, the early vocabulary exposure indirectly affected grade four reading comprehension.

Group book reading in preschool also had long-term associations with later reading. Conversations which included analysis of stories and discussion of words and teacher corrections of incorrect responses predicted receptive vocabulary at the end of kindergarten. This enhanced vocabulary ability was associated with better vocabulary in fourth grade. Also, preschool teachers' efforts to hold a child's attention was related to fourth grade comprehension skills.

"While raising the level of interaction in group activities is important, some of my stronger results in this study are seen from informal interactions between teacher and child, showing the importance of elevated language during times such as play and lunch," he said.

He says parents should carefully examine the nature of interactions happening at their child's preschool to see if teachers are engaged in conversations that will stretch language and knowledge.

Dickinson noted in recent years preschool has become more of an academic setting, where previously the focus was primarily on socialization and kids' adjustment to groups.

The authors also found long-term effects of the home, as children whose parents reported providing more support for early literacy had stronger vocabulary scores in fourth grade. Finally, the structural complexity of children's language at age three was associated with fourth grade vocabulary.

Dickinson reviewed research on preschool interventions in the August 19, 2011 edition of Science and examined the role of adult support for language and challenges preschool interventions face when seeking to foster language growth.

Dickinson will begin working with Nashville, Tenn., preschool programs in August 2011 to develop an approach that can help provide teachers with skills to effectively build language among their students. Specifically, he will examine how teachers can use discussions surrounding book reading in combination with teacher support for dramatic play to build language abilities.
